NOTE: Important Membership Information
- Starting in 2009, attendees of an ESA annual meeting do not become ESA members by default.
- Membership will need to be purchased separately and can be done while registering for the ESA 2009 meeting.
- The individual and corporate membership fees do not include the annual meeting attendance.

On-site registration will be available as of Wednesday, May 20, 2009, 2:00 p.m.
If you have not yet registered for the meeting, please download the registration form, complete it and bring it to the on-site registration desk, located in the Constitution Foyer at the Grand Hyatt Washington.

To qualify for the Early or Late registration fee, your payment -- not your payment commitment -- must be postmarked by the applicable due date.

The option to sign up for 2009 ESA Membership is available during the annual meeting registration process (register online or by completing the meeting registration form, see above).
PAYMENT OPTIONS
All payments must be in U.S. dollars. If not using credit card, all checks must be drawn on a U.S. bank in U.S. dollars. Please make checks payable to ESA 2009. If you prefer to mail your registration, please download the registration form from above, complete and print it and send with payment to Centennial Conferences. If you require alternate payment option, please contact Centennial Conferences.
REGISTRATION CHANGES
Requests to change a conference registration must be made in writing and emailed or faxed to Centennial Conferences (contact information below).
CANCELLATION REQUESTS
For cancellations, pre-paid registration fees are refundable less a U.S. $100 processing fee if notification of cancellation is received in writing (mail, email, or fax) by Centennial Conferences by Friday, April 24, 2009. A 50% refund of the pre-paid registration fee will be given if cancellation request is received between April 25 and May 10, 2009. Cancellations after May 10, 2009 will result in forfeiture of the entire registration fee; however, a substitute registrant will be accepted. Membership fees are non-refundable.
PRE-REGISTRATION CUT-OFF
After Sunday, May 10, 2009 please register on-site.
Pre-registration will be closed at midnight, MST on Sunday, May 10, 2009. Mailed, faxed, or emailed registration forms must be received by May 10, 2009 in order to be processed as a pre-registration and for the late fee to be applied.
